Enrollment Tool
I designed an enrollment tool for caregivers and disabled clients to help them through an extremely complicated enrollment process. This new tool replaced a paper system that had required many iterations of costly back-and-forth with clients to get accurate information from them.

My Role
This tool was designed to completely overhaul an outdated process. I was the UX and UI designer who helped envision the new approach.
I broke down all the elements of the old forms to recategorize the information gathered into easier-to-understand sections. This streamlined the time and effort spent tracking down everything from names and addresses to tax information and how caregivers get paid. By guiding the user through a step-by-step navigation, we eliminated mistakes and missing information and saved precious time.
I also used best practices for all the form fields to make them much easier for the user to complete.
